Ultrasound has been used to evaluate nail diseases in psoriasis [94–96]. A transducer with a frequency of 6–18 MHz and Doppler frequency 7–14 MHz can be used. Wortsman  devised a classiﬁcation system (Table 1). Changes of both Wortsman types have been noted in 50 % patients with cutaneous psoriasis and also 50 % in those with PsA [94, 96]. A signiﬁcant difference was also noted in the mean distance between ventral plate and osseous margins of the distal phalanx. Power Doppler ultrasound detected in the DIP joints and nail beds were also noted in a signiﬁcant number of PsA patients .
The same group subsequently looked at hip arthritis and found similar results, suggesting that Doppler reliably estimates vascularity of synovial tissue . In assessment for synovitis, ultrasound has proven better than both conventional radiography and clinical examination by many investigators [5, 8, 14]. In fact, many studies have shown ultrasound to be on par with MRI in detecting synovitis  while one found ultrasound superior to MRI in detecting this tissue thickening . With tenosynovitis, ultrasound is overall better in diagnosis than MRI.
